Home Health Services

We offer Skilled Nursing, Physical Therapy, Occupational Therapy, and Speech Therapy at home to support independence with compassion and expert care. Services are provided based on eligibility and physician order.

Who Qualifies

A patient may qualify for home health services if they meet the following criteria:

• Medically necessary skilled services are required, such as skilled nursing or therapy
• Homebound status, meaning leaving the home requires considerable and taxing effort (as defined by Medicare guidelines)
• Intermittent skilled care is needed, not continuous or long-term care
• An individualized plan of care is established and periodically reviewed by the physician
• Services can be safely provided in the home setting
• Under the care of a physician who orders home health services

Examples of Qualifying Conditions

• Recent hospitalization or surgery
• Chronic illness with recent decline
• Wound care needs
• New or changed medications requiring monitoring and/or education
• Mobility, balance, or strength limitations
• Speech, swallowing, or cognitive changes affecting safety
• Need for caregiver education related to a medical condition
